1/2/2024 EnchantaVaughn Approved QMHPA greeted Ms. Brown upon arrival to the home setting. Upon arrival to the home setting, Ms. Brown appeared to be in a positive mood evident by her overall disposition. QMHPA inquired about her day. Ms. Brown informed QMHPA that she was in a good mood. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Matthews current thoughts, feelings and emotions. QMHPA engaged Ms. Brown in a discussion surrounding her medication management. QMHPA engaged the client in a discussion to assist her in developing an awareness of her depressive thoughts and actions as it relates to her diagnosis. QMHPA informed Ms. Brown that the purpose of this activity was to assist her with reducing stressors that causes her to become depressed. QMHPA conducted another skill building activity to assist Ms. Brown with developing healthy coping skills that minimize her tendency to isolate. QMHPA informed Ms. Brown that the purpose of this session was to encourage her to implement these into her daily living QMHPA taught Ms. Brown 3-4 positive alternatives to practice to reduce depression and anxiety. Ms. Brown appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. Ms. Brown maintained a positive attitude throughout todayâs session, displaying minor anxiety, distraught responses and apprehensive composure in regards to accomplishing goals in accordance to her treatment plan. During the skills building activity, Ms. Brown was able to explain that coping thoughts are especially helpful when you first notice that youâre feeling agitated, nervous, angry, or upset. Ms. Brown agreed with QMHPA and expressed that she did not use self-encouraging coping thoughts or self-monitoring to assist her with handling distressing situations.
